A Benefit That Can Give Caregivers a Break

UP TO $2,625 IN ANNUAL RESPITE CARE SERVICES

That's equivalent to up to 79 hours of in-home care based on a $33/hour care rate. Respite care can give family caregivers time to rest, run errands, attend appointments, or simply recharge while their loved one receives support.

 

What Support Can You Receive?
The GUIDE Program can provide:
  • Dementia-trained nurse support with regular check-ins
  • 24/7 access to a care team
  • Care coordination with doctors, specialists, and other healthcare providers
  • Medication review and support
  • Family education and one-on-one caregiver coaching
  • Connections to valuable community resources
  • Annual respite care services for qualifying families
